Birth Center Birth: Pain Management, Hospital Transfers & Costs Explained

If you’re considering a birth center birth, three questions almost always come up: How will I manage the pain? What if I need to transfer to the hospital? And what will it actually cost? In this episode of All Things Women’s Health, Dr. Chris Stroud and Marianne Stroud, CNM, tackle all three — with the transparency that Holy Family Birth Center in Fort Wayne, Indiana is known for.

Pain Management Without an Epidural

One of the most important shifts in mindset for birth center families is understanding that labor pain is not the same as traumatic pain. Rather than signaling that something is wrong, labor pain signals progress — and that reframes everything about how you cope with it.

At Holy Family Birth Center, pain management is built on a foundation of one-on-one continuous support, complete freedom of movement, and access to water — whether that’s laboring in the shower or giving birth in the tub. Nearly 80% of births at Holy Family happen in or around the tub, a testament to how effective hydrotherapy is for pain relief and relaxation.

Additional tools include IV pain medications, nitrous oxide (a helpful bridge for moms approaching full dilation), and local anesthetics when needed for repair. But the cornerstone of it all is support — from a dedicated midwife, a birth team, and an involved partner who has the space and encouragement to play an active role.

What Happens If You Transfer?

Transfers from the birth center to the hospital are uncommon, but they do happen — and Holy Family handles them differently than most. Whether a transfer occurs before labor, during labor, or after delivery, your care team from Fertility & Midwifery Care Center comes with you. The same physicians and midwives who have been with you throughout your pregnancy continue your care at DuPont Hospital. You are never handed off to strangers.

The most common in-labor transfer reason is prolonged labor without progress — not an emergency, just a need for additional support. Emergency transfers are exceedingly rare.

The Real Cost of Birth Center Birth

The professional fee for full prenatal, birth, and postpartum care through FMCC is $3,415 — billed to insurance the same way regardless of where you deliver. The facility fee depends on whether your insurance is in-network (carriers include Cigna, Anthem BCBS, Sagamore, and PHP) or out-of-network ($5,000, with prepay discounts available). Health sharing ministries such as Samaritan and Christian Healthcare Ministries are also welcome and have historically covered birth center births at 100%.

For many families — especially those with high-deductible plans — birth center birth is significantly more affordable than a hospital birth, with a better experience to match.
